Due to the busy schedules of our panel physicians and the additional efforts required of Excel staff and those of the physicians, the following policies will apply:

Any rescheduling or cancellation prior to 2 weeks of a scheduled evaluation will require a $75 service fee per occurrence.

Any rescheduling less than 2 weeks prior to the scheduled evaluation will require a $150 rescheduling fee.

Any cancellation less than 2 weeks prior to the scheduled evaluation will require a $250 fee.*

* Some physicians may require a higher fee for cancellations.
